A
FEW OF THE THINGS THAT WE CAN DO FOR YOU

You may enroll in the Corporate Services Health Insurance Plan when
you become eligible. Corporate Services will pay half the premium
for covering you, up to $40 per month. The remainder will be deducted
from your paycheck. You may choose to add coverage for your family,
which you will pay yourself.
The
Benefit: Please check with the office nearest you for the current
benefits available to you.
Eligibility: To become eligible, you must have worked 400 hours
in four consecutive months. Once you have become eligible and choose
to join the Plan, we will start deducting your share of the premium
from your paychecks. Your coverage will start the following month.
To remain eligible, you must work at least 100 hours per month;
if you do not, you must pay the entire premium yourself.

When
you have worked 1,000 hours in a calendar year, you have earned
an attendance bonus of 16 hours. When you reach 1,600 hours, you
have earned another 8 hours, with another 8 hours earned at 2,000
hours. Regular and overtime hours are added together. The attendance
bonus will be paid at the average straight time for the year to
date. To receive your attendance bonus, look at the "Hours"
box in the "Year-To-Date" section of your paycheck stub
each week. When it reaches 1,000, 1,600, or 2,000 hours, ask for
your bonus.
